Licensed CC BY-NC-ND 4.0

Those who open source or share their work Creative Commons are a passionate and collaborative bunch.  We work hard to distribute our work for the benefit of the community but within a licensing model with basic rules delimiting appropriate use including proper attribution, rework and re-distribution.

In simple terms:

You are free to share — copy and redistribute the material in any medium or format.

We cannot revoke these freedoms as long as you follow the license terms:

Attribution — You must give appropriate credit AND provide a link to the license. You may do so in any reasonable manner, but not in any way that suggests the licensor endorses you or your use.

NonCommercial — You may not use the material for commercial purposes.

ShareAlike — If you remix, transform, or build upon the material, you must distribute your contributions under the same license as the original.

Here are 2 examples:

1. Here's how I state the license in the Maturity Model itself and give kudos to those who have contributed factors or changes to the base version:
This work licensed Creative Commons Attribution-NonCommercial-NonDerivatives 4.0 International. The model is part of the Slimgim family of templates attributed to Paul Giroux.

This model is based on the hard work of our peers. Their factors are cited on the 'References' sheet.  A special thank you to those who have contributed both factors, feedback and improvements to the base model: Patrick Wilke-Browne (State of Iowa) as well as the Slimgim-T (Slimgim for Transportation) Team consisting of Eric Abrams (Iowa), Van Colebank (Tennessee), Linda DeMasi (Arkansas), Michael Green (Volpe), Allen Ibaugh (Urisa), Brett A Juul (Oregon), Ian Kidner (Ohio), Kim McDonough (Tennessee), Christopher Meredith (Utah), Drew Quinton (Volpe), Mark Sarmiento (Volpe) & McKenna Stahl (Volpe).

2. Here is how a modified, adapted and improved project - Slimgim-T (Slimgim for Transportation) presents the licence:
